Unit Description: Provides professional nursing care to patients in varying state of health and illness by assessment, planning, implementation, and evaluation of the nursing plan of care. The Staff Nurse II functions as an essential member of nursing and multidisciplinary teams, providing direct patient care via phone and MyChart triage.
Position Specifics: This position entails the below:
- Supporting Neurology’s multidisciplinary clinics at Fanny Allen each Thursday, for patients with ALS and Muscular Dystrophy diagnoses
- Collaborate with on-site team of:
- Neurologist
- Physical Therapist
- Occupational Therapist
- Respiratory Therapist
- Speech Language Pathologist
- ALS Association Coordinator
- Social Worker / Mental Health Counselor
- Connecting with ALS patients before and after clinic for pre-clinic assessment and post-clinic treatment recommendations
- Facilitating flow of patients through their clinic day
- Telephone triage to support this same population
- When there is a clinical need, supporting additional Neurology patients with telephone triage
Experience: Greater than one year. Ambulatory triage or Neurology experience preferred.
