VT LPN or RN licensure required. Shifts vary based on department need. LTC experience preferred. Must possess a high aptitude for learning, strong customer service skills, and teamwork skills.
LPN/RN Sign-on Bonus
An experience-based sign on bonus: $5,000-$7,000 for LPNs and a $5,000-$10,000 for RNs (subject to applicable taxes) is available for this position! Please note that current UVM Health employees are excluded from eligibility for this bonus.
Charge nurses will also receive a $3/hour shift differential
Post Acute at Helen Porter Rehabilitation & Nursing
The goal of the Post Acute Unit at UVMHN Helen Porter Rehabilitation & Nursing is to provide supportive, goal-directed care following surgery and/or hospitalization. Specially designed treatment programs provide patients with the preparation, support and education that enables them to return home as quickly as possible. Our goal is to help patients reach their highest possible level of independence. A partial list of conditions we work with includes strokes, neurological conditions, orthopedic care, respiratory conditions, and cardiac and post-trauma situations
Routine Therapy services are provided Monday through Friday. We also have the capability of providing therapy six days a week if that is what is required to maximize positive results. Nursing staff provide the necessary extension of therapies when therapy staff are not present to provide direct service. This may include walking, exercises and supervision of other activities set up by the therapy staff.
Charge Nurse Role:
Hours: 8am-4:30 pm
Days: Monday through Friday
Purpose:
The Charge Nurse works under the guidance of and in collaboration with the Post-Acute (PAC) Unit Manager. The Charge Nurse is responsible for assisting the PAC Manager in planning, implementing, and evaluation of nursing practice on the PAC unit. Under the guidance of the PAC Manager, the Charge Nurse is expected to provide clinical as well as administrative leadership for the unit. This includes creating an environment that provides patient care that is efficient, safe and of a high quality. The Charge Nurse is expected to work collaboratively with staff, physicians, and other administrative personnel for all units at Helen Porter Healthcare and Rehabilitation Center (HPHRC).
Job Relationship:
Reports to the Unit Manager and the Director of Nursing

Educational Requirements:
- Graduate from an accredited school of nursing.
- State of Vermont Registered Nurse (RN or LPN) license required
- Demonstrates knowledge and clinical competence in clinical area of responsibility

Duties and Responsibilities:
- Provide supervision of the nursing staff
- Orients new employees within guidelines established by the unit’s professional organization and for bargaining unit members under the terms of the CBA
- Provides clear expectations for unit personnel based on standards of care
- Maintains the integrity of nursing by respecting the rights and confidentiality of patients, families, and health care providers
- Maintains a safe environment for patients and staff
- Verifies availability of all supplies required for correct operation of the department
- Facilitates the successful implementation of the nursing process on his/her unit
- Provides leadership and direction in implementation of strategic goals and objectives.
- Demonstrates effective conflict resolution skills
- Supports an environment that fosters open communication among all health care team members
- Provides direct and timely feedback to members of the health care team
- Promote collaboration and teamwork through staff coaching and mentoring
- Assist with clinical assessments and interventions as needed
- Serve as a point person with any significant change in condition such as falls or conditions that result in death or transfer to an acute care facility
- Communicate important information to next shift charge nurse
- Communicate facility issues, concerns or grievances to the appropriate contact person
- Initiate investigation of resident or family member grievances
- Interpret, support, and enforce facility policies and procedures
- Participate and support quality improvement initiatives as assigned
- Provide patient care within scope of RN license as determined by VT State Board of Nursing
- Patient Assessments
- Coordinate patient admissions and discharges
- Wound care
- Collaboration with outside agencies to facilitate care
- IV/Central line care
- Medication administration within scope of RN license as determined by VT State Board of Nursing
- Care Plan writing and chart reviews
- Medicaid/Medicare documentation
- Other duties as assigned
