Social Worker (LMSW/LICSW) – Home Health

If you are looking for a Monday through Friday 8 hour work day social worker position, look no further.

⭐ Education & Licensure

  • Master’s Degree in Social Work (MSW) required

  • Vermont LMSW required; LICSW preferred

  • At least 2 years of relevant experience (healthcare experience preferred)

Key Skills & Qualifications

  • Strong communication, counseling, and rapport-building skills

  • Ability to assess psychosocial needs and explain medical information clearly

  • Experience with crisis intervention, community resources, and care management

  • Strong knowledge of local community resources

  • Knowledge of Medicare/Medicaid, insurance processes, and mandated reporting

  • Reliable transportation and valid driver’s license (travel required)

  •  Ability to work collaboratively with patients, families, and interdisciplinary teams

What You’ll Do

  • Complete comprehensive in-home psychosocial assessments

  • Support patients and families in coping with illness and disability

  • Coordinate care plans with medical team and community partners

  • Provide crisis intervention, short-term counseling, and education

  • Document assessments and care plans according to regulatory guidelines

  • Participate in team meetings, care conferences, and quality improvement activities

Working Conditions

  • Work in patient homes, community settings, and varied environments

  • Occasional exposure to emotional situations or environmental stressors
