⭐ Education & Licensure
Master’s Degree in Social Work (MSW) required
Vermont LMSW required; LICSW preferred
At least 2 years of relevant experience (healthcare experience preferred)
Key Skills & Qualifications
Strong communication, counseling, and rapport-building skills
Ability to assess psychosocial needs and explain medical information clearly
Experience with crisis intervention, community resources, and care management
Strong knowledge of local community resources
Knowledge of Medicare/Medicaid, insurance processes, and mandated reporting
Reliable transportation and valid driver’s license (travel required)
Ability to work collaboratively with patients, families, and interdisciplinary teams
What You’ll Do
Complete comprehensive in-home psychosocial assessments
Support patients and families in coping with illness and disability
Coordinate care plans with medical team and community partners
Provide crisis intervention, short-term counseling, and education
Document assessments and care plans according to regulatory guidelines
Participate in team meetings, care conferences, and quality improvement activities
Working Conditions
