Lead the Way in Hospice and Palliative Care

Are you a passionate and results-driven healthcare professional looking to make a significant impact on the quality of a patient’s end of life care? Join our dedicated team as a Hospice Liaison. This pivotal role requires a strong leader who is knowledgeable and passionate about hospice and palliative care to coordinate and drive business growth while fostering collaborative relationships with key healthcare providers and community members.

Key Responsibilities:

  • Referral Generation and Management: Proactively identify, develop, and maintain strong relationships with referral sources such as hospitals, skilled nursing facilities, and community organizations.
  • Transition of Care Coordination: Ensure seamless and efficient transitions of care for patients, working closely with the clinical management team and other stakeholders.
  • Industry Knowledge and Education: Stay abreast of industry trends and best practices in hospice and palliative care. Educate healthcare professionals and community partners on our services.
  • Strategic Partnerships: Build and nurture strategic partnerships with key stakeholders to enhance our market position.

Qualifications:

  • Essential:
    • Current RN licensure or compact licensure recognized by the State of Vermont
    • Minimum 5 years of experience in the hospice field
  • Preferred:
    • Hospice & Palliative Care Certification (or willingness to obtain within 1 year)
    • Advanced training in Serious Illness Conversations (e.g., TalkVermont)
    • Experience in sales, marketing, and liaison roles

Requirements:

  • Current RN licensure recognized by the State of Vermont required.
  • Appropriate experience in specific clinical area. Varies by unit.
