Join our dynamic team as a Care Manager, where you’ll play a pivotal role in managing the care and services of diverse patient populations. Your mission will be to ensure optimal utilization of healthcare resources, working closely with interdisciplinary teams to achieve the best patient-centered outcomes. You’ll proactively identify and address barriers to treatment, health, and wellness, serving as a crucial link between patients, healthcare teams, payers, and the community. Additionally, you’ll contribute to mentorship, training, and continuous process improvement within your team.

This position will support UVM Health Network Primary Care Clinics in the Berlin, VT area. This role is hybrid, allowing for remote work and requiring an onsite presence in clinics 2-3 days per week.

KEY RESPONSIBILITIES

  • Coordinate comprehensive care and services across the continuum of illness.

  • Promote effective utilization and monitoring of healthcare resources.

  • Collaborate with interdisciplinary teams to achieve optimal clinical and resource outcomes.

  • Identify and intervene in barriers to treatment, health, wellness, and prevention.

  • Act as a liaison between patients, healthcare teams, payers, and the community.

  • Participate in mentorship, training, and process improvement initiatives.

EDUCATION REQUIREMENTS:

Social Work Pathway:

  • Master’s degree in Social Work from an accredited institution.

  • LICSW certification preferred.

  • Case management accreditation (CCM, ACM, or ANCC) preferred.

Nursing Pathway:

  • Registered Nurse (RN) license in the State of Vermont.

  • BSN strongly encouraged for current internal RNs.

  • Case management accreditation (CCM, ACM, or ANCC) preferred.

Occupational Therapy Pathway:

  • Bachelor’s or higher degree from an accredited Occupational Therapy program.

  • Licensed or eligible for licensure in Vermont.

  • Case management accreditation (CCM, ACM, or ANCC) preferred.

Physical Therapy Pathway:

  • Bachelor’s or higher degree from an accredited Physical Therapy program.

  • Licensed or eligible for licensure in Vermont.

  • Case management accreditation (CCM, ACM, or ANCC) preferred.

EXPERIENCE:

  • 2-3 years of clinical experience in a healthcare setting.

  • Prior experience in care management or care coordination is required.
