Pharmacist Clinician - Transitions of Care

JOB DESCRIPTION

The transitions of care pharmacist clinician will work directly with the Working to Reduce Admissions Program (WRAP) service at the University of Vermont Medical Center to identify and resolve medication related problems (MRPs) that may be contributing to repeated admission for vulnerable patients. The pharmacist will work closely with case management to identify patients who would benefit from a pharmacist assessment and interventions. The pharmacist will complete an admission medication reconciliation for newly screened patients, coordinate with the primary team and case management during admission to help facilitate safe medication plans upon discharge, and complete post-discharge medication reconciliations. The clinician should act independently to identify the best course of action for a patient and set up necessary consultations with other healthcare providers to facilitate care. Additional responsibilities toward patient care may include reviewing and confirming medication histories through contact with outpatient pharmacies and interviews with patients and their families, pre-discharge counseling, post discharge telephone calls, documentation in the patient's electronic health record, and liaison with several disciplines including the patient's primary inpatient team, primary care provider, sub-acute rehabilitation centers, home health services, and outpatient pharmacies. 

The clinician will serve as a liaison to the WRAP service and pharmacy leadership and contribute to data monitoring, data collection and quality improvement efforts around the service. As an inpatient pharmacist clinician, the transitions of care clinician will be expected to provide staff continuing education, precept pharmacy students and residents, and serve on the Residency Advisory Committee. They will generate quality improvement or research ideas in their area of specialty annually to precept or conduct individually or as a team. They will serve as a lead for any pharmacists or technical staff who may also work with the WRAP service.

The clinician will contribute to the culture of safety to submit and review SAFE events as discovered. They will work to help address systematic issues around transitions of care to improve these systems for all patients. The clinician will also assist with reviewing and updating workflows around medication transitions of care for the department and within Epic.

EDUCATION

Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience. Licensed

pharmacist in the State of Vermont.

EXPERIENCE

3 years of advanced practice experience in their area of expertise or equivalent experience.

Knowledge/Special Skills:

Excellent written, verbal and interpersonal skills required. Excellent organizational and team building skills. Ability to work effectively as part of a clinical care team. Ability to exercise sound professional judgement, set priorities, make critical decisions and respond quickly to emergent requests. This position requires a high degree of specialized knowledge of clinical therapeutics within their area of expertise. Frequently applies a high degree of analytical, interpretive and evaluative skills as part of clinical care responsibilities.
