JOB SUMMARY:
The Transitions of Care Ambulatory Pharmacist Supervisor oversees the daily operational workflow of the Meds to Beds program, the transitions of care staff and all associated services. The Transitions of Care Ambulatory Pharmacist Supervisor will develop and implement operational policies designed to identify and address medication access barriers and liaison with available programs, resources, and members of the care team to help facilitate a smooth transition out of the hospital, promote discharge therapy adherence and help prevent non-adherence related readmissions. The Transitions of Care Ambulatory Pharmacist Supervisor will be responsible for implementing the Meds to Beds program in in units throughout the medical center and work directly with unit management to ensure program success and operational efficiency. The Transitions of Care Ambulatory Pharmacist Supervisor will be responsible for overseeing clinical triage and resolution of ambulatory prescription concerns for both the retail and Meds to Beds operation as well as continuous quality improvement program implementation. The Transitions of Care Ambulatory Pharmacist Supervisor will work with the Manager, Outpatient Pharmacy to develop and implement wraparound services. This role is based out of the Outpatient Pharmacy.
EDUCATION:
Pharmacy Degree from ACPE accredited college of pharmacy (five-year B.S. or Pharm. D.).
Certifications: Licensed and registered to practice pharmacy in Vermont.
EXPERIENCE:
2 to 5 years related work experience required.
One year minimum retail pharmacy within past 5 years, one-year minimum experience in a health system pharmacy. Equivalent alternate pharmacy experience is acceptable.
PREFERRED QUALIFICATIONS:
2 to 5 years of experience in Transitions of Care.
ASHP Accredited Residency in Pharmacy Practice (PGY1).
Experience in a hospital outpatient pharmacy.
BPS Specialty Certification.
