Case Management and Social Work at UVM Medical Center is hiring a Case Management and Social Work Lead to join their team!

Please note: This is an onsite position at UVM Medical Center in Burlington, VT.

JOB DESCRIPTION

The incumbent is responsible for the organization and management of the coordination of services between the Case Management department and community partners. They build and maintain mutually beneficial relationships, facilitate communications, and coordinate activities between the Case Management department and our community partners. The incumbent seeks to establish mutually beneficial, encouraging relationships with targeted groups, establish parameters for collaborative efforts, and serve as a reliable contact point with whom stakeholders can engage. Additionally, they will conduct periodic evaluations with the leadership team to elucidate additional possibilities for collaboration.

  • Ability to assess and intervene in care progression and transitions.

  • Skilled in mediation, negotiation, and conflict resolution.

5. Clinical Expertise & Regulatory Knowledge

  • Utilizes clinical skills to support care for patients with complex medical needs.

  • Maintains knowledge of regulatory and legal requirements (e.g., HIPAA, JCAHO, CMS).

  • Coordinates care conferences and collaborates on therapeutic plans.

6. Organizational & Planning Skills

  • Demonstrated ability to organize and manage coordination of services.

  • Skilled in devising frameworks for maximizing partnership benefits.

  • Maintains knowledge of partner norms, processes, and goals.

7. Professionalism & Compliance

  • Upholds organizational mission, values, and standards of excellence.

  • Adheres to professional appearance and conduct policies.

  • Demonstrates commitment to confidentiality, compliance, and safety standards.

EDUCATION

Master in Social Work from an accredited school of Social Work.  LICSW preferred.

OR

State of Vermont Registered Nurse (RN) license required.

OR

Master's degree from an accredited institution in a Health or Human Services field.  Licensure preferred.

Case management accreditation required by a nationally recognized accrediting body for case management (examples: CCM, ACM or ANCC certifications) preferred.

EXPERIENCE

Minimum of 3 years’ experience in hospital case management preferred.
