The Population Health Services Organization (PHSO) is hiring for three full-time Clinical Resource Coordinator - LPNs to join our Annual Wellness Visit (AWV) team!

Please note: This hybrid position allows our LPNs to work remotely from their home office while conducting telehealth Medicare Annual Wellness Visits (AWVs) for UVM Health Primary Care patients. The role also requires periodic onsite presence for orientation and training sessions.
 

WHO WE ARE

The UVMH (University of Vermont Health) PHSO (Population Health Services Organization) is a physician and payer partnered operation that provides the right constellation of services (e.g. care management and care coordination, data and analytics, provider reporting, Quality, Risk Adjustment, and Value-based Contract Performance, etc.), that enables the health network to successfully perform in High Value Care and Value-Base Contracts.

JOB SUMMARY

The PHSO Clinical Resource Partner works under the general direction of the AVP Population Health Services and works closely with Population Health Services Organization (PHSO) and Medical Group teams in support of High Value Care (HVC) performance initiatives and centrally supported clinical workflows. These initiatives are data-driven and will be assigned by the Network, AVP Population Health Services in collaboration with Network VP Clinical Operations. The PHSO Clinical Resource Partner will maintain working knowledge of HVC and PHSO strategy and primary care clinical workflows.

Patient-Centered Care & Engagement

  • Conduct Annual Wellness Visits (AWVs), close care gaps, and facilitate screenings.

  • Communicate effectively with patients and care teams to enhance coordination and follow-up.

Clinical Knowledge & Practice

  • Function within the scope of LPN licensure as defined by the Vermont State Board of Nursing.

  • Apply clinical protocols for documentation, referrals, and problem list updates.

Data Management & Documentation

  • Utilize EMR and registry systems to manage patient data and care gap closure.

Team Collaboration & Communication

  • Work closely with PCP practices, care teams, and PHSO staff.

  • Share data, support pre-visit planning, and respond to inquiries from practices.

  • Participate in team meetings and contribute to a collaborative care environment.

Training & Education

  • Meet organizational competencies and participate in ongoing education.

  • Serve as a clinical role model and mentor for new staff.

  • Support orientation and training efforts within the PHSO team.

Technical Proficiency

  • Experience with EMR systems and Microsoft Office applications (Outlook, Excel, Word).

  • Strong time management, multitasking, and written communication skills.

EDUCATION

Graduate of accredited Licensed Practical Nursing Program. Current LPN licensure or compact licensure recognized by the State of Vermont is required.

EXPERIENCE

One to two years of experience in relevant health care setting. Primary Care experience preferred. Proven customer service experience required.
