The Population Health Services Organization (PHSO) with UVM Health Network is hiring for a Resource Coordinator to join their team in the Central Vermont area!
This position is hybrid. Allowing for remote work and requiring an onsite presence at our Central Vermont clinics in Washington County, VT 2-3 days per week.

The UVMHN (University of Vermont Health Network) PHSO (Population Health Services Organization) is a physician and payer partnered operation that provides the right constellation of services (e.g. care management and care coordination, data and analytics, provider reporting, Quality, Risk Adjustment, and Value-based Contract Performance, etc.), that enables the health network to successfully perform in High Value Care and Value-Base Contracts
JOB DESCRIPTION:
Coordinates the care and services of selected patient populations across the continuum of illness, with a focus on addressing the Social Determinants of Health; promotes effective utilization and monitoring of health care resources; and assumes a role with the interdisciplinary teams to achieve optimal patient-centered, clinical and resource outcomes. Works within care teams to address barriers to treatment, health, wellness, prevention, improvement and outcomes. Serves as an important link between the patient, the healthcare teams, the payors and the community.
EDUCATION:
Associates Degree in Human Services. Bachelor’s level degree in Human Services preferred. Candidates with high school diploma and relevant work experience in a healthcare setting may be considered.
EXPERIENCE:
2-3 years of experience in a healthcare setting (or equivalent)
Care Coordination experience highly preferred. Customer Service experience preferred
