Training (up to 6mos) will occur in person at our Colchester office. Candidate MUST be able to attend training in person during this time. Remote hybrid opportunity after successful onboarding and orientation!!!

The Intake and Referral department is the heart of Home Health & Hospice operations, and we want you on our team! We have an open position for a registered nurse who is interested in supporting an exceptional patient experience, every time. Our registered nurses and team assistants work together to make certain we have all required information and documentation for a complete referral, and coordinate with referring providers and internal directors and managers to support patient access to our services. This position offers an opportunity for a registered nurse to use their clinical experience to evaluate patient eligibility for our services and support patients, their families, community members and referring providers in understanding the breadth and depth of the services we offer.

Minimum Qualifications:

ASN or higher degree in nursing required.
At least 3 years of nursing experience.
Current RN licensure or compact licensure recognized by the State of Vermont required.


Duties:

• Assessment of customer needs using clinical knowledge and comprehensive understanding of the UVM Health Network - Home Health & Hospice services.

• Accepting referrals from doctors, hospitals, patients or families, and other agencies

• Developing and researching information in order to complete the referral. Use assessment skills to determine that all confirmed referrals meet defined criteria for admission to Home Health & Hospice

• Receiving written and verbal orders from physician practices and notifying UVMHN Home Health & Hospice staff in a timely and accurate manner

• Documenting complete referral information into clinical computer systems of the Home Health & Hospice. Use of UVM Health Network systems for research and development of complete referrals made by UVMMC.

• Keep accurate documentation and log for all referrals processed

• Serve as resource to staff regarding patient information, community resources and regulatory requirements

• Provide ongoing education for referring providers, patients, their families and community members regarding regulations for all insurance payers

• Create the insurance note in clinical computer system of the UVMHN Home Health & Hospice and share with A/R department

• Assure verification of uninsured clients and initiation of process to provide uninsured clients with funding source

• Review, develop, and complete electronic documentation for all off hours (evening/night/weekend and holiday) referrals, working quickly to provide timely information for the admitting teams. This may include working extended hours on Monday and the day after a holiday

• Verify with every referral that MD is active

• Confer regularly with supervisor, liaison nurses, home visiting staff and other Home Health & Hospice personnel to plan and coordinate care

• Research questions and collaborate with all customers to resolve problems and facilitate initiation or progress of home care

• Good understanding of the nature of home health nursing and a strong interest in the field

• Other duties as assigned by supervisor
