Join our team as an Interventional Radiology Care Coordinator to deliver and coordinate exceptional care for our IR patients! As an ambassador for IR and key voice to the leadership team, you will collaborate with the IR procedure and clinic teams, as well as network, hospital, and community services to meet the needs of our diverse patient population. This dynamic role involves engaging in patient and family education, ensuring continuity of care, participating in quality improvement projects, and more.
Ideal candidates are experienced RNs with strong leadership, communication, and organizational skills, ready to make a meaningful impact in patients' lives. Experience in IR and familiarity with the University of Vermont Health Network, the University of Vermont Medical Center, and community resources are preferred.
The IR Care Coordinator is a day shift, salaried position, with no call requirement.
ABOUT INTERVENTIONAL RADIOLOGY AT UVM MEDICAL CENTER:
The inpatient interventional radiology team at The University of Vermont Medical Center (a primary stroke center and level 1 trauma center) performs procedures related to vascular access, palliative treatment, traumas, thrombectomies, thrombolysis, cancer treatments, strokes, and pain injections – just to name a few!
The Interventional Radiology clinic team provides consultation, education, pre and post procedure follow up services to patients receiving Interventional Radiology procedures. The clinic IR team welcomes nurses that bring a strong commitment to collaboration with interdisciplinary teams to provide therapeutic and family-centered care, as well as the ability to apply new skills to a dynamic workload.
RELOCATION ASSISTANCE:
Candidates joining us from a distance that is greater than 50 miles from their previous role and home are eligible for relocation assistance up to $9,200.
JOB DESCRIPTION:
The Care Coordinator is responsible for coordination of activities related to a specific patient population and the administration of direct patient care for a specific unit and shift. The Coordinator functions as a leader of multidisciplinary teams, providing and coordinating care to patients and families. Concurrent utilization and service review, consultation, patient and family education, promotion of continuity of care, preceptorship and committee work are required at this level of practice.
EDUCATION:
Current RN licensure or compact licensure recognized by the State of Vermont is required. The University of Vermont Medical Center is moving toward an all Bachelor of Science in Nursing (BSN) workforce. All external candidates hired into, or current employees promoted to, a RN position will be required to obtain their BSN as outlined in the terms of their offer letter. Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.
