JOB DESCRIPTION:
We are seeking a compassionate and organized Dialysis Social Work Case Manager (SWCM) to join our team at the South Burlington Dialysis Unit, part of the UVM Health Network. This role offers the opportunity to build meaningful, ongoing relationships with patients while balancing their emotional and psychosocial needs with practical resource support.
As the primary and only social worker on-site, you will work closely with an interdisciplinary team of physicians, nurses, dietitians, and dialysis technicians, collaborating to provide holistic, patient-centered care. You will have the support of a supervisor and other social workers in similar roles at other dialysis units across the state. You will play a crucial role in supporting patients as they navigate the challenges of End Stage Renal Disease and dialysis, from emotional adjustment to connecting with community resources.

- Conduct comprehensive psychosocial assessments and provide therapeutic interventions tailored to individual patient needs.
- Advocate for patients and their families, helping them access vital resources and community services.
- Support patients emotionally as they adjust to dialysis and life changes, offering counseling, education, and practical guidance.
- Coordinate closely with the interdisciplinary team to address barriers to treatment adherence and improve health outcomes.
- Assist patients in managing the transition between hospital care and outpatient dialysis, ensuring continuity of care and support.
- Promote patient empowerment, health maintenance, and self-advocacy through direct support and education.
This is a dynamic, relationship-driven position that blends clinical social work and case management skills. EDUCATION:
MSW from an accredited school of social work required. Licensure through the State of Vermont is preferred.
EXPERIENCE:
Greater than 1 year experience working in a human service organization, health care setting, or managed care system is required.
