Are you a compassionate and skilled professional dedicated to supporting individuals and families through grief and loss? Home Health & Hospice is seeking a Bereavement Care Coordinator to provide essential direct services and coordinate bereavement programs in line with National Hospice and Palliative Care Organization (NHPCO) standards. In this vital role, you'll offer comfort, guidance, and resources to those navigating the complexities of grief.
What You'll Bring
We're looking for someone with a strong background in counseling, a deep understanding of grief, and a commitment to serving our community.
I. Education:
- A Bachelor's degree in Social Work, Counseling, or a related field is required.
- A Master's degree is preferred.
II. Experience:
- A minimum of two years of relevant experience in grief and loss, family therapy, and group facilitation.
- Possess a strong base of clinical knowledge and skills relevant to grief and loss, family therapy, and group activities.
- We also value a combination of education and experience that demonstrates comparable knowledge and skills.
What You'll Do & How You'll Make an Impact
As our Bereavement Care Coordinator, you'll be at the heart of our bereavement services, ensuring compassionate and comprehensive support.
Performance Standards:
- Collaborate effectively as a fully integrated member of our interdisciplinary team.
- Coordinate Bereavement Plans of Care for clients and oversee follow-up by staff and volunteers.
- Work as an effective member of the bereavement team under the supervision of the Bereavement Team Lead.
- Organize and participate in bereavement care plan conferences and interdisciplinary meetings.
- Participate in the creation and delivery of educational programs on loss, grief, and related topics for families, volunteers, and staff as needed.
- Participate in the annual Hospice Remembrance Ceremony.
- Facilitate bereavement support groups, as needed.
- Provide consultation to Interdisciplinary Team members and staff on bereavement counseling as needed.
- Offer education and referrals to the broader community not directly served by UVMHN McClure Miller Respite House, as needed.
- Perform other duties as assigned by your supervisor.

- Travel will be required.
